It took me some hours to get this investigated by DirectAdmin, but they found out that is was a bug in OpenLiteSpeed. The fix will be included in OLS 1.8.5, that I've just successfully tested.

After the change I haven't seen issues, so that's a success. I'm still hoping to find answers to my additional questions, to understand it better.
